Sounds of Honor Audiology is dedicated to maintaining the privacy of your protected health information (“PHI”). In the course of providing hearing healthcare services under our HonorCare™ framework, we create and receive records containing your health information. This Notice explains our legal duties and privacy practices with respect to that information, and describes your rights regarding your PHI.
We are required by law to maintain the privacy of your PHI, to provide you with this Notice of our legal duties and privacy practices, to abide by the terms of the Notice currently in effect, and to notify you if a breach of your unsecured PHI occurs.
How We May Use and Disclose Your Health Information
- Treatment. We may use and disclose your health information to provide, coordinate, or manage your audiological care — for example, sharing test results with a referring physician or an ENT specialist involved in your care.
- Payment. We may use and disclose your health information to obtain payment for services, including billing your insurance carrier, Medicare, TRICARE, or VA Community Care Network (CCN), and verifying eligibility or benefits.
- Health Care Operations. We may use and disclose your health information for operational purposes such as quality assessment, staff training, licensing, and business planning necessary to run our practice.
- Appointment Reminders & Care Follow-Up. We may contact you by phone, text, mail, or email to provide appointment reminders, hearing aid maintenance reminders, or information about HonorCare™ follow-up services.
- Required by Law. We will disclose your PHI when required to do so by federal, state, or local law.
- Public Health & Safety. We may disclose PHI to prevent or lessen a serious and imminent threat to health or safety, or as required for public health activities.
- Health Oversight, Legal Proceedings & Law Enforcement. We may disclose PHI to health oversight agencies, in response to a court or administrative order, or to law enforcement under limited, legally defined circumstances.
- Military and Veterans. If you are a member of the Armed Forces, we may release PHI as required by military command authorities or in connection with VA benefits determinations.
- Workers’ Compensation. We may disclose PHI as authorized by workers’ compensation or similar programs.
Other than the uses above, we will not use or disclose your health information without your written authorization. This includes most uses and disclosures of psychotherapy notes (not applicable to routine audiology records), marketing communications, and the sale of PHI.
